Phases of Application

Starting an ARMHS clinic is a multi-phase process that usually takes 9-15 months to be completely capable of providing services.  Below is the 4 main phases that every ARMHS clinic will need to go through to provide services.

Phase 1: Setting up your business.

  1. Registering your business with the MN secretary of state
  2. Setting up your FEIN and MN State Tax ID
  3. Setting up a business bank account
  4. Setting up your company’s NPI2
  5. Applying for General Liability Insurance

Phase 2: Applying for ARMHS Certification from MN Department of Human Services (DHS)

  1. Owners attend the MN-Trainlink ARMHS Admin Class
  2. Clinician attends the MN Trainlink ARMHS Clinical Class & completes all required pre-requisites
  3. Complete DHS-7181 Application
    1. Fill out general business information
    2. Fill out Treatment Supervisor information
    3. Attach Admin Sections A-C: Policies
    4. Attach Admin Section D: Mock File
    5. Include signed DHS-7181a form
  4. Upon a successful review, you will receive an email from DHS with your “ARMHS First Approval”.
  5. Apply to MN counties that your business intends to provide ARMHS in.
  6. Upon approval, you will then receive an email from DHS with your “Final ARMHS Approval.”

Phase 3: Applying to MN Health Care Programs (MHCP) & State Medical Assistance (MA)

  1. Complete the MHCP Application for both your Company and Treatment Supervisor
  2. Pay the MHCP Application Fee (Currently $631)
  3. Pass the OIG unannounced Site visit
  4. Upon successful completion of the OIG Site visit, your company will receive MHCP Approval and it’s MN-ITS login to access MA

Phase 4: Applying to Private Insurance for your Company’s Contract and Clinician’s Credentialing with the following relevant providers

  1. Blue Cross Blue Shield – (Metro & Greater MN)
  2. Health Partners – (Metro & Greater MN)
  3. Hennepin Health – (Metro only)
  4. Optum/Medica/United Health – (Metro & Greater MN)
  5. UCare – (Metro & Greater MN)
  6. Itasca Medical Care – (Itasca County only)
  7. Prime West Health – (South East MN only)
  8. South Country Health Alliance – (Greater MN only)
